Add a new parameter CarpetInterp::barriers. If set, CarpetInterp
introduces MPI_Barrier statements. These statements catch calling
errors where not all processors call CarpetInterp at the same time.

Add parameters max_core_size_MB and max_memory_size_MB, limiting the
maximum size of core files and the maximum amount of allocated memory.
The limits are set via setrlimit during startup.

Rename cycle, flip, and fill to cycle_all, flip_all, and fill_all,
respecively, in the ggf class. Remove the c ("component") argument.
Instead, loop over all components. This mirror earlier changes to
sync etc.

When passing in a pointer to mem<T>, also pass the size of the
pointed-to memory region, so that mem<T> can check that there is
enough space.
Keep track of the number of allocated bytes in mempools.

Add parameter printstatus. Output statistics only if desired.
Add parameter legacy_init as new initial parallelisation choice.
Add parameter ignore_initial_overhead, which ignores the first time
the loop is encountered when optimising the parallelisation.
Correct serious inconsistency when choosing the tiling.
Output average loop time as well.

Thorn InitBase specifies how initial data should be calculated. There
can be three possibilities:
init_some_levels: Set up at least one time level; other time levels
are scratch space
init_single_level: Set up exactly one time level; other time levels
are not acessed
init_all_levels: Set up all active time levels
Carpet checks this parameter and ensures that this setting is
consistent with Carpet's parameters
init_each_timelevel
init_fill_timelevels
init_3_timelevels
The new parameter init_fill_timelevels copies the current time level
into all past time levels, very similar to what
MoL::initial_data_is_crap does.

the corresponding write call of this attribute fails because the attribute's scalar dataspace was already closed.
