These tests have all been replaced by tests using EinsteinExact, which
is more accurate and less sensitive to roundoff error.

exact duplicate, since the original test used features of Exact to fix
the time derivative of the shift inside the BH that is not implemented in
EinsteinExact. I thus had to switch of the evolution of B. If we later
modify NoExcision to fix this, we can turn the evolution of B on again.

Introduce cctk_ash, describing the process-local array shape that has
been allocated. This may be larger than cctk_lsh, the process-local
shape that should be used.
Retire cctk_lssh and related infrastructure to handle staggered grid
functions.
